What is a Change Request Form?

A Change Request Form is a document used to request changes to piece of software. It includes the reason for change, priority, and cost. In general, SCR items are coordinated between the project manager, software manager, and the testing manager.

Purpose of a Change Request Form

The Change Request Form template helps Software Test managers to:

• Change Request # – capture the CR number
• Reason for Change – describe the reason for the change
• Assessed By – identify who assessed the change item
• Priority – set the priority of the change item.
• Impacts – describe the impact of the rest of the system
• Effort – estimate how much effort this will costs
• Hours – the number of hours this will take
• $ – the cost per hour
• $ Total – the total cost for the project
